This is the easy evaluation form.  There are only three questions. Thanks for filling this out, it really helps me focus on improving the class.

Please note that this form is not a substitute for the official course evaluation you will do near the end of the semester. But it is a way of telling me what I'm doing right and wrong as the semester proceeds. That way I can fix things when I still have the opportunity!
