Completed forms may be submitted by one of the following methods:
- Submitted electronically by hitting the SUBMIT button for the PDF file
- Emailed as an attachment for the WORD file to the following email address: KALP-L@LSV.UKY.EDU
- Printed (either PDF or WORD files) and mailed to

A special note about the Application Forms
Although the Adobe Acrobat versions for the forms have been set up so that you should be able to save your file and then re-open them to edit, we suggest you try this before going too far. If you have problems, please let us know.
