Completed forms may be submitted by one of the following methods:
- Submitted electronically by hitting the SUBMIT button for the PDF file
- Emailed as an attachment for the WORD file to the following email address: KALP-L@LSV.UKY.EDU
- Printed (either PDF or WORD files) and mailed to

A special note about the Application Form
If you choose to use the Acrobat (pdf) version of a form, you will not be able to start the application, save it, and return to complete complete the form later unless you have a full version of Adobe Acrobat or other pdf editor program.
We have discovered that older versions of Word have had problems using the Word version of the applications. We have created a second version of the form that should work for Word 97 through Word 2002 (Word XP). We have also discoverd a submit problem for people who have Acrobat Reader and do not have Acrobat Standard or Acrobat Professional. We are working to fix that problem as quickly as possible.
