Weeds of Kentucky Crops
When submitting plant samples for identification:
  1. Completely fill out information requested on Weed Identification Form

    To Access [Weed ID Form]   <requires Adobe Acrobat 5.0 or higher>

    Note:  Some fields must be completed before sending:

  • Name of the person who submitted the sample

  • Date submitted

  • Signature

  • County identifier

  1. Save completed form to your computer with a new file name [SAVE FORM AS button].

  2. Print completed form to mail with sample [PRINT FORM button].

  3. Open your E-mail program with E-MAIL FORM button.

  4. Attach a saved electronic copy of the completed WEED IDENTIFICATION FORM and send to the following E-mail address WeedID@uky.edu.  A diagnostic response will be returned electronically.  NOTE:  Electronic images of the plant can also be attached to the email submission of the completed form.
