Applications must show clearly the relationship of the proposed activity: (1) to a recommendation from the most recent Program Review cycle reflected in the Program Review Implementation Plan, (2) to the assessment plan for the program(s) and (3) must show how the information gained will advance student academic outcomes assessment for the program(s).

APPLICATION FORM FOR 2006/2007 QEP PROJECT FUNDING

Address each of the following in a brief proposal.

2. Purpose: Give a brief statement about what you intend to do. Describe the general improvement strategy.

3. Project Methodology: Briefly explain how you plan to carry out the project. Identify criteria and strategies you will use to track your progress on this effort.

4. Student Learning Outcome Focus: Identify the unit’s student learning outcome(s) addressed in this project.

5. Faculty Involvement: Describe how faculty, beyond those responsible for the project, will be involved.

6. Timeline: Provide a timeline that identifies major activities, from beginning to completion, in monthly intervals.

7. Relevance to Unit’s Assessment Plan: Briefly explain how this project relates to 1 or more recommendations (strategic planning priorities) featured in the most recent Program Review Implementation Plan and the related assessment plan for the program(s).

8. Significance Or Impact Of The Project: Describe the expected benefits of the project--how the information gained will sustain and/or advance student learning outcomes and related assessment work for the program(s).

9. Budget: In addition to outlining a budget plan for major expenditures indicate if a budget carry-over into a 2nd budget cycle is anticipated, along with a brief justification.
