The Graduate Program in Communication: Forms

Funding for Conference Travel

Independent Study Agreement
University Scholar Form

M.A. Forms
Ph.D. Forms

All completed forms -- with appropriate signatures -- are to be submitted to the Associate Dean for Graduate Programs in Communication which are then routed to the University of Kentucky Graduate School.

Students first enrolled in a doctoral program in the fall 2005 semester are required to take the qualifying examination within five years of entry into the program. Extensions up to an additional three years may be requested. Extensions up to 12 months may be approved by the Dean of the Graduate School upon receipt of a request from the Director of Graduate Studies. If the qualifying examination has not been passed at the end of five years, or at the end of all approved time extensions the student will be dismissed from the program.

Students who are planning to complete doctoral qualifying exams in the fall semester need to complete the Recommendation for Qualifying Exam Form within the first six weeks of the fall semester (no later than the first week of October or the first week of February). Students can then defend any time during the first semester of enrollment. In addition, students must register for 767 for 2 hours of residency credit (which is considered full time). Under the new policy students will only need to complete two continuous semesters of the 767 (2 hours per semester or 4 hours for the two semesters of 767 post-qualifying residency credit) before they can graduate.

After the qualifying exam, students are required to complete two mandatory continuous semesters of residency credit in order to meet graduation requirements.

  • Dissertation Proposal Defense Form (Submitted as soon as possible after the successful completion of the qualifying exam--usually within the first semester of residency--but after the successful defense of the qualifying exams)
  • Graduate Studies Degree Card (Submitted early in the semester in which the dissertation will be defended--can be as early as the second semester of simultaneous residency.)
  • Submission of the next form requires a minimum of eight weeks in order to provide sufficient time for the UK Graduate School to procure an external examiner--who is to be consulted BEFORE determining the date, time, and place of the final dissertation defense.
  • Notification of Intent to Schedule a Final Doctoral Exam (Allows UK Graduate School sufficient time to arrange for Outside Examiner)
Submission of the next form requires a minimum of four weeks. Students should provide two weeks for a majority of the committee members to read and approve the scheduling of a final dissertation defense. The UK Graduate School requires that BOTH the Dissertation Approval Form and the Request for Final Doctoral Exam be submitted two weeks before the defense date.
  • Dissertation Approval Form (Submitted along with the Request for Final Doctoral Exam Form AFTER the final draft of the dissertation has been approved by the Director of the Dissertation and the UK Graduate School has procurred the Outsider Examiner.)

Doctoral students must allow two weeks for a majority of their committee members to read the final draft of the dissertation so that a majority of the committee members can approve the scheduling of the final dissertation defense.

University of Kentucky Graduate School policy requires that the dissertation approval form "must be signed by the majority of the advisory committee, including the director of the dissertation." If the committee has four members, the director and two other members must sign (total of 3). If the committee has five members, the director and two other members must sign (total of 3). If the committee has six members, the director and three other members must sign (total of 4).

The approval sheet does not provide final approval of the dissertation--it only allows for the scheduling of the final dissertation defense.

  • Request for Final Doctoral Exam (Submitted at the same time as the Dissertation Approval Form only AFTER the Outside Examiner and all other committee members have agreed on a date, time and place for the final dissertation defense.)
