Applying for Rank Change
Applying for a rank change, an endorsement, extensions, for school media librarian: Candidates who are applying for advanced certification or for additional certificates will fill out the EPSB's TC-1 form, the accompanying UK College of Education advanced certification form, and a UK transcript request form. Paper versions are available in 166 TEB, or can be printed from the web sources below:

The candidate should request a copy of the completed Program of Study from the SLIS School Media Program Coordinator, Prof. Melissa Gardner. After receipt of the Program of Study and completion of all forms, all documents should be mailed to the Academic Services and Teacher Certification office (166 TEB).
