You can now fill out all forms from your keyboard. Enter information into
user-friendly form fields, print the document and submit it to the appropriate
office for any required signatures.
We are working to make departmental (EDC) forms available in both Microsoft Word
Rich Text Format (RTF file extension) and Adobe Portable Document
Format (PDF file extension). Graduate School (GS) forms are
available in PDF format only. If you can work with MS Word, we
recomend RTF forms if you wish to save the document with the
information you have entered into form fields. Adobe PDF
documents open with the free Adobe Acrobat Reader, but you will not be able to
save the document unless you have the full version of Adobe Acrobat.

Save it. Print it. Read it! If you lose your copy, you can
always come back here for another one.
- GS Doctoral
Advisory Committee Request Form
Doctoral students are assigned an advisor for the first 18 credit hours of
graduate work. Upon completion of these 18 credit hours, students must
choose a dissertation director and form a committee. In addition to
the chair, your committee typically includes two additional graduate faculty
members chosen from the Department of Curriculum and Instruction and one additional
graduate faculty member chosen from another department within the College
of Education. This core committee should remain unchanged for the duration
of your program.
- Curriculum Contracts
Your next step is to work with your committee to plan the remaining coursework
and to plan for your qualifying examination.
- Instruction and Administration, Mathematics Education
- Instruction and Administration, Instructional Systems Design
- Instruction and Administration, all other areas
- GS Request for
Qualifying Examination
If you plan to receive residency credit hours for EDC 749 or 769, you
must pass the qualifying exam within six weeks of the beginning of the semester
or within three weeks of the beginning of the second summer session.
- GS Preparation
of Theses and Dissertations
You are now an official doctoral candidate and you're ready to start
writing. These guidelines are updated periodically. Be sure you
are working from the most recent edition.
- GS
Notification of Intent to Schedule a Final Doctoral Examination
Submit this form eight weeks in advance of the proposed week of your
defense. Do not indicate an exact date and time. The graduate
school will search for a suitable outside examiner. You will then work
with your core committee and your outside examiner to set the exact date and
time of your defense.
- Now you are prepared to move ahead to graduation.
