Graduate School Forms


The Department and programs have adopted a number of forms for administrative purposes. On this page we have collected the several forms and reproduced them in several different styles, whenever possible. The forms are available in Word or PDF, where noted. Open the form of choice and use the "Save As" command to save the Word file to your desktop. To make sure you are printing the least number of pages, you might want to "view print" and adjust the margins before printing.

Departmental forms

  • Application form (PDF) to be submitted to the Director of Graduate Study (DGS)
  • Doctoral Program Plan (Word) to be submitted to the DGS
  • Planned Fifth-Year Application forms to be submitted to the DGS and Graduate School
  • Rank II and Rank I Application forms to be submitted to the DGS and Graduate School
  • Doctoral Application forms to be submitted to the DGS and the Graduate School
  • Four (4) Letters of Recommendation form (PDF) an autobiography, statement of professional goals, sample of scholarly writing, and any other appropriate documents to be submitted to the Director of Graduate Study.
  • Incomplete ("I") Grade Record (MSWord) to be completed by the instructor and the student, if possible. The document is filed in the Department Office.
  • Independent Study Contract (form) must be signed by the faculty member and placed in the student's DGS file.
  • Transfer of Credit (MSWord) to be completed by the advisor and submitted to the Director of Graduate Study for review and submission to the Graduate School.

Graduate School forms The forms require Adobe Acrobat Reader. If you have a question about a form contact the Director of Graduate Study (DGS).

All graduate programs require an application to the Graduate School and an application to the Director of Graduate Study in the Department of Educational Leadership Studies. A package of application materials is preferred.
