Graduate School Forms


The Department and programs have adopted a number of forms for administrative purposes. On this page we have collected the several forms and reproduced them in several different styles, whenever possible. The forms are available in Word or PDF, where noted. Open the form of choice and use the "Save As" command to save the Word file to your desktop. To make sure you are printing the least number of pages, you might want to "view print" and adjust the margins before printing.

If you are applying to a program you must submit both a Departmental application and an application to the Graduate School.

Departmental forms

  • Application form (PDF) to be submitted to the Director of Graduate Study (DGS)
  • Planned Fifth-Year Application (PDF) forms to be submitted to the DGS
  • Rank II and Rank I Application (PDF)forms to be submitted to the DGS
  • Doctoral Application (PDF) forms to be submitted to the DGS
  • Three (3) Letters of Recommendation form (Word) an autobiography, statement of professional goals, sample of scholarly writing, and any other appropriate documents to be submitted to the Director of Graduate Study.
  • Incomplete ("I") Grade Record (MSWord) to be completed by the instructor and the student, if possible. The document is filed in the Department Office.
  • Independent Study Contract (form) must be signed by the faculty member and placed in the student's DGS file.
  • Transfer of Credit (MSWord) to be completed by the advisor and submitted to the Director of Graduate Study for review and submission to the Graduate School.
Graduate Program in Rehabilitation Counseling forms
  • Application for Field Placement (Word) to be submitted to the fieldwork faculty supervisor for the semester you plan to complete your fieldwork
  • Application for Fieldwork Examination (Word) to be submitted to the Program Director. To be eligible for this examination student must file the application in the Departmental Office within 30 days after the beginning of the semester or 15 days in the Summer Session in which they expect to take this examination. Admission to Field Work Examination is normally taken during a student's second semester of full time study or after the student has completed at least 20 hours of rehabilitation counseling coursework.
  • Program Final Examination (Word) indicates the type of examination and to be submitted to the Program DGS
  • Consent for the Transfer of Records (PDF) allows for the Program to send your academic records to CORE and is submitted to the Program DGS to be submitted to the Program DGS.
  • Learning Goals Contract (Word) to be completed by the practicum instructor and the student. The document is filed in the Program Office.
  • Weekly Summary of Fieldwork Activities (Word) must be signed by the fieldwork supervisor and placed in the student's DGS file.
  • Transfer of Credit (Word) to be completed by the advisor and submitted to the Director of Graduate Study for review and submission to the Graduate School.

Kentucky Teacher Certification The application for Kentucky teacher certification forms may be obtained in 166 TEB. Questions should be directed to Martha Geoghegan (859) 257-7681, myrt@uky.edu

Graduate School forms The forms require Adobe Acrobat Reader. If you have a question about a form contact the Director of Graduate Study (DGS).

All graduate programs require an application to the Graduate School and an application to the Director of Graduate Study in the Department of Curriculum of Instruction. A package of application materials is preferred.
