Evaluation application forms


The department of Educational Policy Studies and Evaluation offers both master's level and doctoral level programs. Depending on your career goals, you will want to apply to:
    1. EPE Master's Degree Programs (2)
    2. EPE Doctoral Degree Programs (2)

In addition to the application packet sent to the Director of Graduate Study, The Graduate School (Gillis Building, University of Kentucky, Lexington, KY 40506-0033) requires:

  1. Graduate School application
  2. Two official copies of transcripts from each institution previously attended
  3. Official score report of the GRE Aptitude Test

Please bear in mind that review of your application by the department won't begin until you've been accepted by the Graduate School. Their turn-around time averages a week to ten days.
