- Declare a Minor form (PDF) - Students planning on declaring a minor must take this form to the department that houses the minor for the advisor’s signature.Once completed, the form must be returned to the College of Fine Arts Student Afairs office.
- Exceptions petitions for USP, UKCore, and Graduation Writing Requirement
- Repeat Options must be returned to the CFA Dean’s Office, 206 Fine Arts Building. According to Senate Rules, A student must be enrolled at UK at the time he/she files the repeat option. - Learning contracts for Independent Study, Coordinate Study, AAD or TA Experiential, Senior Projects and/or other Internships must be completed, including professor and chair signatures, and turned in PRIOR to registering for these courses.
