- Complete and submit an application for registration at New Registration Form (pdf) or stop by the C.S.I. and pickup a New Registration Form.
- Develop and submit a Constitution and submit By-Laws if needed.
- Identify a minimum of five students (Undergraduate, graduate, part-time, or full-time) on the form.
- Officers must be currently enrolled students in good academic and financial standing with UK.
- Identify a faculty member to serve as an advisor for your organization. Must be an Associate, Assistant, or Full Professor.
- Submit copies of local, state, and national constitutions and bylaws of affiliations.
- Must have a University Advisor complete their information, if required.
- Turn in all completed paperwork to the CSI Administrative offices for processing.
