- You are checking on your eligibility to participate in a benefit plan (or, see page 3 of the Benefits booklet to review employee eligibility for various benefit plans, and page 4 for health plan availability by county).
- You want to find out if a plan is available to you.
- You want to change your address.

Visit our Employee Self-Service page for a guide to the address change feature. - You want to change your beneficiary, level of coverage, file a claim or if you want to request a summary plan description (for life and AD&D insurance only).
- You have questions or issues regarding online enrollment or paper enrollment.

Call the plan administrator (see telephone numbers or e-mail addresses listed on the back/last page of the Benefits booklet) if:
- You would like a summary plan description of your plan.
- You have questions about the status of a claim.
- You want to find out if a provider is available in a particular plan.
- You have questions about coverage for a specific service or condition.
- You need help finding a primary care physician and/or participating dentist.
- You would like to request a list of participating providers.
