In general, enrollment is not required unless:
- You need to add, drop or change your level of benefits coverage.
- You need to start or continue child/dependent care or medical care flexible spending accounts (FSA).

If you answer YES to any of the questions below, you need to re-enroll.
Flexible Spending Accounts
- Do you wish to participate in the health and/or dependent care spending plan(s)?
Health Plans
- Are you currently enrolled in a health plan and would like to do any one of the following:
- change to another health plan? (See what's new for 2011.)
- add or remove your spouse/sponsored dependent or dependent(s)?
- If you are NOT currently enrolled in a UK health plan, would you like to enroll?
Dental Plans
- Are you currently enrolled in a dental plan and would like to do any one of the following:
- change to another dental plan?
- add or remove your spouse or dependent(s)?
- drop dental coverage?
- If you are NOT currently enrolled in a dental plan, would you like to enroll?
Vision Plan
- Are you currently enrolled in a vision plan and would like to do any one of the following:
- add or remove your spouse or dependent(s)?
- drop vision coverage?
- If you are NOT currently enrolled in a vision plan, would you like to do so?
