Vision Plans - Eligibility

Employees may enroll in the vision plan and cover their eligible dependents even if they are not enrolled in a UK health plan. However, the vision plan eligibility requirements the same as the requirements of the health plans: 

Employee Eligibility

  • regular full-time employee (FTE)
  • regular half-time employee
  • regular part-time employee, with an assignment of .20 FTE or more
  • temporary part-, half-, or full-time with an assignment of at least .20 FTE and with sufficient earnings to make the necessary premium payments
Dependent Eligibility
  • your spouse
  • your sponsored dependent
  • your dependent children up to age 26
  • children of your adult sponsored dependent up to age 26
  • your mentally or physically disabled children of any age who were or became disabled before the age limit and who were already covered by your plan
