As a new employee, there are many forms that must be completed on or before your first day of work. Your supervisor will assist you with obtaining and completing these forms.
Required forms include:
- I-9: Be sure to bring your driver's license and original, unlaminated social security card or passport. If you do not have these documents, please check the following site, which lists other acceptable documents: www.uky.edu/HR/maind/i9's.html
- Federal Tax Form
- State Tax Form
- Direct Deposit: Be sure to bring a deposit slip or voided check indicating where you want your income to be deposited
- Drug-Free Workplace Agreement
