Participation in Healthtrac Rewards is easy!
The UK Healthtrac Rewards Program is a partnership among your University Health Plans, UK Health & Wellness Program, and The Good Health Company. This program is designed to help you become more aware and involved in improving your health while earning up to $180 per year for your participation. Until now, health plan members have only been able to earn rewards by participating in the Healthtrac Rewards program online.
To encourage more health plan members to participate and enjoy this benefit, we are pleased to announce a paper option is now available to retirees. You can either participate on-line OR you can choose to participate using the paper Health Assessment (HA).
Retiree Spouses
If your spouse is currently enrolled in your UK Health Plan, he or she is eligible to participate in either option of the Healthtrac Rewards Program. If interested in the paper option, call 859-257-9355 (WELL) or 800-941-9804 for details. For the web based option, follow the instructions below.

NEW users this is how you make good health pay:
- Call (859) 257-9355 to get started. We'll sign you up and give you instructions.
- Simply visit www.healthydashboard.com/UK
- Login by typing in your user name and password.
- If you get a message that says you have successfully logged in that's all you need to do unless you have a Health Assessment* to fill out.
All Web-Based Users:
Be sure you meet all three steps listed below to earn your maximum reward for the current 6-month incentive period (October through March or April through September.)
1. Visit your personal health page and complete a minimum of 1-2 Health Assessments* (HA) in the current 6-month period. Reminder: the URL address is now www.healthydashboard.com/uk.
*Step 1 earns $30 every 6 months
2. When you complete your HA, check the box “Yes, a Health Coach can contact me.”
*Steps 1 and 2 earn $60 every 6 months
3. You must improve or maintain your Overall Risk Score based on the comparison of your 2 most recent health assessments (1-2 of which must be completed in the current 6-month period of October through March or April through September). Your Overall Risk Score is calculated and determined by your health habits, health confidence and overall view of your health, as reported on your health assessment.
*Steps 1, 2 and 3 earn $90 every 6 months
You must complete each step to be eligible for the entire $90 bi-annual reward. If you skip a step, you will earn the reward amount at the last step you completed.
Your reward check will be sent to the home address you provide when you complete your HA. You will receive your check in the months of October and April each year.
*The Health Assessment should not take the place of your regular physician visit and should only be used for general health awareness

1. Complete the Health Assessment* and return it to The Good Health Company in the enclosed business reply envelope. Your results will be generated and mailed back to you within 7 business days. You will receive a follow up paper HA in the mail every 3-6 months, which must be completed to earn the amount indicated above. (You must complete 1-2 Health Assessments in the current 6-month period.)
