Retirees: Participation is easy!

Important: New users may register any time, but can't start earning rewards until the Health Assessment is completed. The next paper or online Health Assessment will be available beginning January 1, 2010.

UK Retiree Spouses/Sponsored Dependents: You may participate in UK Healthtrac Rewards either online OR via the paper Health Assessment. The steps to earn your reward are different based on which option you choose:

 


Web-Based Healthtrac Rewards

Four Simple Steps to Make Good Health Pay

1. Complete the Health Assessment when it is offered twice per year – during July through September and January through March. You will receive $25 for each completed Health Assessment – up to $50 per fiscal year.

 

Important: You MUST complete at least one Health Assessment before you can begin earning any rewards for that fiscal year. (For example, if you didn't complete the Health assessment by September 30, 2009, you will not be able to begin earning any rewards until you complete the next available Health Assessment in January.)  

 

2. Enroll in and complete either a BeH.I.P. telephonic health coaching program or an online Healthy Living program and earn a one-time payment of $30 per fiscal year. You will receive a reward for only ONE of these programs each fiscal year. (Click here for more details on these programs.)

 

3. Complete the assigned Health Center Quiz and earn $10 each quarter. You will be notified which quiz to take for the quarter in the monthly Healthtrac reminder e-mail.

The Health Center Quiz for this quarter is Weight. Click on Health Centers and then Weight. Select your answers and click Submit. Complete it by September 30!

4. Log in to your Healthtrac home page and earn at least 500 Wellness Credits per month using the Health Activity Tracker. (Link found on Your Home Page under “Your Toolbox.”) This step earns $5 per month.

 

Web-based users: Visit https://ukhealthtrac.online.staywell.com and click Register Now. You will be asked for your first name, date of birth, and the last four numbers of your social security number to register. (For more details on registering, click here.)

Paper-Based Healthtrac Rewards Program

Who is eligible for the UK Healthtrac Rewards Paper Option?
UK retirees and their spouses/sponsored dependents may choose the paper Health Assessment option.


How do I sign up for the Paper Option?

Beginning January 1, you may call StayWell at 1-800-926-5455 to request your paper Health Assessment for the January through March 2010 period.

Can retirees and their spouses/sponsored dependents switch from paper to online participation (or vice versa) during the fiscal year?

No. For example, if a retiree or their spouse/sponsored dependent begins the fiscal year using the paper option, he or she must continue participating via paper until the end of the fiscal year. However, he or she may switch to the other option at the beginning of the next fiscal year.

What steps do I take to earn my full reward of $180 per fiscal year using the paper option?
1.
Call StayWell at 1-800-926-5455 to request your paper HealthPath Health Assessment, during the two designated UK Healthtrac Rewards assessment periods (July through September and January through March). Complete the assessment and return it by the applicable postmark deadline of either September 30, 2009 or March 31, 2010. You will receive $40 for each completed Health Assessment – up to $80 per fiscal year.

2. When you receive your HealthPath Health Assessment results, call 1-800-926-5455 and speak with StayWell Health Coach to review your results. You will receive $25 for the completed call to a Health Coach – up to $50 per fiscal year.

3. Call 1-800-926-5455 to enroll in a BeH.I.P. health coaching program. Complete the program and earn $50 once per fiscal year.
