What is UK Healthtrac Rewards Program?


UK Healthtrac Rewards Program is a partnership among your University Benefit Plans, UK Health & Wellness Program, and The Good Health Company. This program is designed to help you become more educated about your health and how to be more involved in improving your overall health.

What is The Good Health Company?


The Good Health Company is a leading health education company with 15 years experience helping more than three million participants make positive changes in their health. In October 2004 The Good Health Company received the "Best Application of Behavior Change Principles to Disease Management" Award from the Disease Management Association of America (DMAA).

Who is eligible?

Employees, retirees, spouses, and sponsored dependents who are enrolled in a University Health Plan (UK-HMO, UK-PPO, UKPPO High, UK-EPO, UK Healthfirst and Indemnity) are eligible for participation. Dependents are not eligible at this time.

How does the program work?

Participation is simple—just follow these easy steps.


To get started, simply visit www.healthydashboard.com/uk and register as a new user. You will be asked to create a user name and password. Your id number is the last 4 digits of your Social Security number. This number will only be used for initial registration.


Step One earns $5 a month: Visit your personal health page every month and complete your Health Assessment (HA) when available (every 3-6 months).


Step One and Step Two earns $10 a month: When you complete your HA, check the box “Yes, a Health Coach can contact me” on your profile page.


Step One, Step Two and Step Three earns $15 a month: After visiting your personal health page, use the Health Activity Tracker to track your healthy activities. For each healthy activity or behavior you engage in you will earn Wellness Credits. You must earn a minimum of 500 Wellness Credits each month to receive the full incentive of $15 a month. All activities must be entered by the last day of the month.


Each step builds upon the previous step; make sure you complete each step to earn the entire $15 a month.

The www.healthydashboard.com/UK site asks for my id number. What is this?

In order to make sure you are a UK employee and currently enrolled in a UK Health plan, Healthtrac must compare your record with a file received from UK. The only way to identify you is by using the last 4 digits of your current Social Security Number. This is the only time you will be asked for any information regarding your SSN and it will only be used to identify you during the login procedure.

How often will I be called if I check “Yes, a Health Coach can contact me?”

You will receive a call from a Health Coach after you complete your HA every 3-6 months.

What is the purpose of the Health Coach calling me?

The Health Coach will contact you to review your letter report and health profile, and also to provide you with information concerning programs offered at the University. You are not obligated to enroll in any programs, you just need to be open to speaking with a Health Coach for several minutes. You will only be contacted every 3-6 months after you complete your HA. If you elect to not have a Health Coach contact you, you will earn $5 a month for your completion of the HA.

I already participate in Healthtrac Rewards Program. Do I need to do anything differently?

If you have been participating in Healthtrac Rewards Program you will need to make sure you follow the new steps to earn your $15 a month. Previously you only had to visit your personal health page and complete the HA every time it was available to earn $10 a month. Now under the new participation guidelines you must complete the three steps to earn up to $15 a month.

What is my personal health page?


Your personal health page is a secure and confidential Web page that provides you with health information. Your personal health page can only be accessed by you with a user id and password you create. This page contains information about how you can improve your health as well as reliable information about a wide range of health topics. Your Health Assessment will be available on this page, as will the reports that provide highly personalized feedback about your health status.

What is the Health Assessment (HA)?


The Health Assessment (HA) is a simple questionnaire that asks about health habits, family history and past medical needs. Typically, the questionnaire takes 5-10 minutes to complete. After you complete the questionnaire, a personalized letter signed by a physician, and a Health Profile will be available on your personal health page. This Personal Health Profile will give you areas of success and areas of improvement along with university resources to help you along the way.

What happens with the information I provide?

Healthtrac enters the information into a secure computer system. The information is strictly confidential between you and Healthtrac. This personal information will be used to generate a personal letter report, health profile and to help the Health Coach find programs to best suit your needs. Personal information will not be shared with a health benefit administrator or used within the University for any other purpose.

So what is the catch?

Over the past two years the University Administration has worked very hard to make health benefits more affordable for the University community. In order to continue this progress, it is important that we work together to control the rising cost of health benefits. The University is demonstrating its commitment to this effort and is asking the entire University community to work toward controlling these costs. By participating in this program you will learn how to improve your health and help the University control costs. So what’s the catch? The only “catch” is whether or not you choose to participate. By choosing to participate, you can improve your health, decrease health care costs
and earn money in return.

I've completed questionnaires before. How is this different?

This Healthtrac program is different because it helps you track your progress over time. Data is linked from one questionnaire to another, so you can see health improvements. Each letter, report, and health profile provides a record of your health history as it relates to the Healthtrac program.

I already see my doctor. Why should I participate?

The Healthtrac program was developed and approved by a group of physicians. The program provides educational materials and personal support with proven results. The materials will help to reduce health risk factors and/or manage certain chronic health conditions. The information is tailored to each individual's personal needs. Participants are encouraged to share the letter and report with their doctor.

In order to control health care inflation within the University, each employee has a responsibility to help manage his/her personal utilization of medical services. The Healthtrac survey allows eligible plan participants the opportunity to monitor their health between physician appointments and to become more active in their health care.

Where can I find the time to participate?

The Healthtrac program is simple and easy. It takes less than 10 minutes to complete the Health Assessment (HA). You may visit your personal health page when it is convenient from any Web accessible computer.

I have not received an email stating my personal report is ready. Why?

If you did not receive your personal report immediately after you completed the HA and you did not receive an email stating your report is ready, please contact the Health & Wellness office at 257-9355.

I have looked at my personal graph report, and the graphs did not appear. Why?

We experienced some technical difficulties initially when the graph reports were released. However, this problem has been fixed; please check your graph again. If you still have trouble, please contact the Health & Wellness office at 257-9355.

How do I check “yes” if I don't have an assessment available?

If you do not have a HA available, then you can go to your profile page and check “Yes.” You need to make sure you have checked “yes a Health Coach can contact me” in order to earn $10 a month.
