Q: Can I view a copy of the survey before logging on to complete it?
A: A basic template of the survey can be viewed using the links below. Your college's or unit's survey may be altered slightly to include customized items or language.

Q: What if I experience problems with the survey link I was provided?

Q: How long will it take to complete the survey?
A: Depending on the length of your open-ended responses, the survey should take between 15 – 25 minutes to complete. Please allow ample time to complete the survey, especially if you wish to respond with lengthy comments.
Q: Can I start the survey then finish it at a later time?
A: You may start and restart the survey whenever you wish. However, please make sure to retain the email in which the survey link was provided. Contact Amy Messer (phone: 257-1633), if you experience any technical difficulty.
Q: What happens if I submit the survey but then decide to edit my responses?
A: We will not replace survey text or responses after the survey has been submitted.
Q: How can I be sure that my confidentiality will be protected when completing the survey?
A: Each user will be randomly assigned a unique survey link (URL), which you will receive via email. The system restricts participation to one response per participant. (Therefore, please do not forward the e-mail to anyone else.) Only one individual who works directly with Vice President Connie Ray will have access to the data that links your email address to your evaluation participation. These data will be immediately discarded at the close of the survey. Please be assured that your responses will be treated in a confidential manner and findings will be reported in summary form only. Q: How will my responses be protected for confidentiality?
A: No cross-tabulations or raw statistics will be reported for any group of demographics with 3 or fewer respondents. Additionally, all open-ended comments will be carefully reviewed and edited to remove any comments that would obviously identify the respondent.
Q: How will the results be reported and to whom?
A: After careful review under the direction of Vice President Connie Ray, a graphical and statistical summary of the responses will be provided to the CAO and supervisor, as well as a verbatim transcript of all open-ended comments. Information about how results will be used by the Dean (or VP/EVP, as appropriate) and the Provost depend on the type of review: annual, formative or summative.
