Examination Information

Exam Request Form

• Have all assignments and previously incomplete assignments submitted.
• Complete and send in the Exam Request Form to the Independent Study Office.

High School

• Have all assignments and incomplete assignments submitted.
• Exams can be taken at your high school under the direct supervision of your counselor.
• Exams can be taken in the Independent Study Office between the hours of 8:30 am and 2:30 pm.
• Have a Picture ID with you.

Middle School

• Have all assignments and incomplete assignments submitted.
• Exams can be taken at your Middle School under the direct supervision of your counselor.
• In addition, you may take the exam in the Independent Study Office with a Picture ID between the hours of 8:30 am and 2:30 pm.
