UK students

Exam Request Form (PDF)

  • Have all assignments and previously incomplete assignments submitted.
  • Complete and send in the Exam Request Form to the Independent Study (ISP) Office.
    Mail the form to:

High School Final Exams

  • Have all assignments and incomplete assignments submitted.
  • Exams may be taken at your high school under the direct supervision of your counselor.
  • Exams may be taken in the Independent Study Office between the hours of 8:30 am and 2:00 pm, but a reservation must be made at least 5 days in advance. To make a reservation, call or email the day and time you want to take the final exam to the ISP office. Call 859-257-4002 or use the information above to email a request for a day and time you want to take the final.
  • Be sure to come with a form of identification that has your picture on it such as a driver's license.

Middle School Final Exams

  • Have all assignments and incomplete assignments submitted.
  • Exams can be taken at your Middle School under the direct supervision of your counselor.
  • In addition, you may take the exam in the Independent Study Office with a Picture ID between the hours of 8:30 am and 2:00 pm, but a reservation must be made at least 5 days in advance. To make a reservation, call or email the day and time you want to take the final exam to the ISP office. Call 859-257-4002 or use the information above to email a request for a day and time you want to take the final.
