The following request form is made available for individuals affiliated with the University of Kentucky Medical Center (UKMC) who want to register for access to the proxy server. If you have not registered for a library card and want to access library resources off-campus, please complete this form. The library bar code assigned to you is your password; the user ID is your last name.

Note: This form is only for UK Medical Center faculty, staff and students who have primary work sites in off-campus locations.
