How do I submit grades?

o Submit On-line.  Log on to the Experiential Education Program (EEP) secure database at link http://iweb.uky.edu/Pharmacy/default.asp.  Click on "Evaluation of Student Performance", select the student, fill out the form, and then submit the form.

o Submit via Fax, or Mail (regular or campus).  You may download a paper copy of the evaluation form to mail or fax at link http://www.uky.edu/Pharmacy/clerkships/GradeForm.pdf.

Why is it important that grades be submitted on time?

It is important that grades be submitted on time throughout the rotation year for a variety of reasons. 
o Timely/Accurate Feedback on Student Performance.  It is difficult for preceptors to provide timely and accurate feedback to the College of Pharmacy and to the students concerned if the grade is submitted late.
o Honors Recognition.  The College of Pharmacy Office of Academic Affairs routinely conducts Grade Point Average (GPA) calculations to determine if students meet the criteria for both College and University academic recognition, to include each semester's Dean's List and graduation honors. 

o  Tuition Reimbursement.  Many students receive tuition reimbursement from their employers; almost all these employers require the students to submit copies of their grades.

How Do I Know If I Have An Overdue Grade?

o Log on to the secure database.
o Click on "Preceptor Information".
o At that web page, you will see a box that looks like this:

o  If there is a "X" in the box for the month you were assigned a student, then the grade was received.  If not, the grade was not received.  

o Notes:  
(1)  If the grade was sent on-line, an "X" will appear in the box almost immediately after the grade is received electronically by the secure database.
(2)  If you send the grade via fax or regular mail, the grade is manually processed and entered into the secure database.  This manual process normally takes 3-5 days after the grade is received.

I thought I sent the grade on-line.  What happened?

You were probably "timed out" while using the secure database.  The secure database has a 20 minute time out security feature (a UK requirement).  When "filling out" the grade form, it is easy to be distracted, receive a phone call, etc and get "timed out".  When using the secure database, if the log on 
window (displayed below) appears after you hit any submit or function type 
                             

button, you were "timed out".  You would be required to log back on.  Depending on the web browser you use, you may or may not lose the info you had previously entered on the grading form. 
o  Note that once you successfully submit the grade form on-line, a window (displayed below) will appear verifying the grade form was submitted.

When are the grades due?

Grades are normally due about 5 working days after the end of the rotation month.  See the table below for more info.
