If you are requesting a transcript to be sent after your August degree has been posted, this will be mailed out after the Board of Trustees confers these degrees at their September meeting.

Instructions for sending by mail or fax. View/print the Transcript Request Form.
You cannot send a Transcript Request Form electronically or via email. Once you have filled out one of the pdf forms below, you must either fax it (859-257-7160) or mail it (see Instructions).
Choose only one of the three options below:
- transcript.pdf - Print the blank form, fill in using pen or pencil, then sign the form (size: 96.4 KB; PDF)
- transcr-form.htm - Print the blank form, fill in using pen or pencil, then sign the form (size: 12 KB)
* Note: Documents in the PDF file format require Adobe Acrobat Reader software. It can be downloaded for free at Adobe.com.
