Minor Application Form: Declaring a Minor
Instructions for using the Declaration of Minor Application Form:
- Students planning on declaring a minor must take this form (PDF format*) to the department that houses the minor for the advisor's signature.
- Once completed, the form must be returned to the minor college/department office listed on the back of this form.
