Residency Application - Documentation Guide Sheet
When completing the Application for Review of Residency Status and deciding which documents to provide, please look through
your completed Residency Application to find information with an (*) asterisk beside it. Each applicable item will need
supporting documentation. (This includes each item in Section IV, Annual Support.) Each case is evaluated on its own
merits. It is the student's responsibility to document all aspects of the case.

- Parent's lease or deed to residence
- Proof of payment of KY state income taxes (pay-stub, W2, etc)
- Proof of loans/financial aid (if applicable)

- Proof of visa status from parent(s) and student (if applicable)

student and spouse both contribute
to household support and are both
financially independent of others:

- Proof of payment of KY state income taxes (pay-stub, W2, etc)
- Proof of loans/financial aid (if applicable)
- Student's KY Driver's License
- Proof of visa status from spouse and applicant
(If spouse was transferred with employer, please include a letter from his/her employer).

- Documentation proving assignment in KY for reasons other than education
- Military ID from student, parent, or spouse

- Documentation proving KY has been home of record since enlistment
- Military ID from student, parent, or spouse
