SIS User Request Form (SIS User Access)

This form must be submitted by your SIS unit representative; it is not for student use! This request must come from your SIS contact person. If you do not know who that is, contact Jodi Davenport at 257-1695.

Please provide the following information for the person for whom you are requesting SIS access:

The person for whom this access is being requested must: fill out, sign, and submit to the address below, the Student Information System Statement of Responsibility (PDF format*).
