View / print a Schedule Building Worksheet. Choose any one of the three options below:

  1. worksheet.pdf -- fill in form using your computer keyboard (size 202.3 KB; PDF format*)
  2. worksheet-small.pdf -- print the blank form, fill in using pen or pencil (size 12.8 KB; PDF format*)
  3. vipform.txt -- small text version of worksheet (size 2.8 KB)

* Note: Documents in the PDF file format require Adobe Acrobat Reader software. It can be downloaded for free at Adobe.com.
