Application Instructions

The requirements for application for the Social Work Ph.D. program through the University of Kentucky are:

  • Completed Graduate School application form. U.K. Graduate School Application (links to UK Graduate School)
  • We require transcripts from all post-secondary institutions you have attended (University of Kentucky graduates are not required to submit UK transcripts). One set of transcripts should be sent to the Graduate School at the following address:

    A second set of transcripts should be sent to:

  • Official transcripts
  • Official report of GRE scores (must be from the last five years).
  • Application fee of $50 for domestic and $65 for International, if applicable

For the Graduate School application form (link maintained by the Graduate School), the appropriate codes are: College - GS, Degree - PHD, and Major - SOWK. These materials should be sent to:

The following materials must be received by the College of Social Work

  •  Application for admission to the Ph.D. in Social Work Program. Ph.D. Program Application (pdf)
  • Three letters of recommendation (Two academic and one from employer). Recommendation letters (pdf)
  • A resume
  • A writing sample. Writing Sample (pdf)
  • A personal statement of career goals, research interests, and reasons for pursuing doctoral studies (no more than 3 pages)
  • A cover letter applying for a full-time student fellowship (if applicable, see the fellowships available)

The admission process involves: (1) Review of application materials by the doctoral admissions committee and (2) personal interviews of those recommended by the committee. It is suggested that you submit your application as soon as possible especially if applying for a fellowship.
