The requirements for application for the Joint Ph.D. program through the University of Kentucky are:

I. The following materials must be received by the Graduate School by February 1.

____ 1. Completed Graduate School application form

____ 2. Two copies of official transcripts from each college/university attended

____ 3. Official report of GRE scores

____ 4. Application fee of $40 for domestic and $55 for International, if applicable

For the Graduate School application form (link maintained by the Graduate School), the appropriate codes are: College - GS, Degree - PHD, and Major - SOWK. These materials should be sent to:

II. The following materials must be received by the College of Social Work by February 1.

____ 1. Application for admission to the Joint Ph.D. in Social Work Program

____ 2. Three letters of recommendation (Two academic and one from employer)

____ 3. A resume

____ 4. A writing sample - (located at the end of the Application)

____ 5. A personal statement of career goals, research interests, and reasons for pursuing doctoral studies
             (no more than 3 pages)

The admission process involves: (1) Review of application materials by the doctoral admissions committee in the institution where you apply, (2) personal interviews of those recommended by the committee, (3) review by the Joint Program Admissions Committee, and (4) review by the Graduate School of the university where the candidate plans to matriculate. It is suggested that you submit your application as soon as possible.
