2. Then press the “Submit Form” button at the bottom of the page. The form can be Submitted Online, Printed and Faxed to the DRC Office (859) 257-1980, or Hand-Deliver it to Room 2 Alumni Gym Disability Resource Center
3. Exams are to be delivered to Lindsay Jansen by 12:00 (noon) the day before the scheduled exam
   Exams can be:
   1. emailed to lbstew2@email.uky.edu
   2. hand-delivered to Room 2, Alumni Gym, Disability Resource Center OR
   3. faxed to Lindsay Jansen at (859) 257-1980

All exams must be e-mailed or dropped off no later than 12:00pm (noon) the day before the scheduled exam
