Forms

University of Kentucky fraternities and sororities are required to submit various information to the Fraternity & Sorority Affairs Office throughout the year. In order to expedite the process, the following forms are useful.

Adobe Acrobat Reader is required to view and print these forms. If you do not have Acrobat Reader on your computer, you may download it free from Adobe's website.

You have two options for completing and returning these forms: 1) You may print them and fill them out by hand; 2) You may save the form to your computer with Adobe Reader, fill it out on-line, print it using your printer and turn it in to 575 POT. Please note that if you do not save the form to your personal computer prior to filling it out, you will not be able to fill it out on-line. The form will remain blank.
