Enter documentation of your immunizations (type and date administered) - REQUIRED

Access on-line health promotion and patient education information

Link to the UK Policy on Mandatory Student Immunizations **
- At a future date, you will be able to make some types of appointments
** Beginning with the Fall 2007 semester, the University of Kentucky requires all incoming students to be immunized before enrollment.
