  1. How do I make a report or referral to the SOC?

    You may report directly to the SOC via the "Student Alert" system in myUK.

    The "Student Alert" report system is accessed via the myUK portal* . Once logged in to the myUK portal you:

    1. Select the "Student Administration" tab and then
    2. Select "Student Alert."
    3. Enter the Student name or ID and hit "Search."
    4. Review the search result(s) and click your intended selection.
    5. Fill out the form* as completely as possible and hit "submit."

    * The "Student Alert" system and the SOC Committee are NOT resources to be used for emergencies!
    If you are in an emergency situation that requires medical,
    psychological or police services, call 911 from a campus phone.
    Off-campus and cell phones, 859-257-UKPD (8573).

    Once the situation is safe, you should submit a "Student Alert."



  2. What if I want to make a report to the SOC but don't have access to the "Student Alert" system?

    If you do not have access to the Student Alert form via myUK, you may phone the SOC Case Manager directly at 859-257-5134.



  3. What should I do if someone has a weapon?

    If you are in an emergency situation that requires medical, psychological or police services, call 911 from a campus phone.

  4. What should I do if someone is threatening to harm themselves?

    If you are in an emergency situation that requires medical, psychological or police services, call 911 from a campus phone.

    • SOC Case Manager

    • Assistant Vice President of Campus Services
    • Assistant Vice President of Student Affairs/Dean of Students

    • Associate Dean of Students for Student Conduct

    • University Health Services Student Behavioral Health Director

  6. How do I reach the SOC Case Manager directly?

    The SOC Case Manager can be reached by phone at 859-257-5134 or e-mail at SOC@lsv.uky.edu.



  7. What do if I need to make a referral after normal business hours?

    After regular business hours, you may call the after-hours cell at 859-576-0871.

    If you are in an emergency situation that requires medical, psychological or police services, call 911 from a campus phone.
