Laptop Checkout Policy
The laptop checkouts are limited to three business days. After one renewal the client will be required to wait for a period of two weeks before checking out another laptop.
If a laptop is returned late, check-out privileges will be suspended for a period of six months.
University of Kentucky/TASC: Faculty Laptop Checkout Policy
I agree to assume full responsibility and fiscal liability for the laptop computer and peripherals issued to me by Teaching & Academic Support Center / AV Services during the period I have it checked out.
I understand that I am required to display a valid UK identification card in order to checkout a laptop computer and that these laptops are only issued to faculty who need the laptop for development of courses, programs and other academic missions of the University.
I understand I may need to justify the need for an extended check out period of over 6 days.
I acknowledge that when the computer is returned, a staff member will verify that all parts are present and that the computer and all peripherals are in good working order.
I have read and understand the University of Kentucky Computing Resource Policy http://ukcc.uky.edu/policy.html and agree to abide by them.
I understand that laptop service may be denied to people who abuse equipment, return laptops late or otherwise disregard the provisions of laptop service.
