The eForms web site provides access to University of Kentucky
forms. There are links to categories of forms, an alphabetic
list of forms by title, and a search eforms function. You
can view, print and complete interactive forms.
Important Information: We've received several complaints that when opened, a PDF form comes up blank. This seems to be an issue with the latest Adobe Acrobat Update 7.0.8.

You can fix this problem easily by refreshing the page. If that doesn't work, clear your browser cache and try again. Use the following directions to clear your browser cache.
1) Internet Explorer: Tools -> Internet Options -> Temporary Internet files -> Delete Files
2) Mozilla Firefox: Tools -> Options -> Privacy -> Cache -> Clear
