Basic Information

The Check In is a free 20-minute integrative health screening to 'Check In' on your health measures and your sense of well-being. During your confidential Check In, you'll connect with a UK Health & Wellness Guide who will support you by completing a comprehensive health screening, listening to you about what area of your well-being matters to you most and then working with you to figure out a next step.

Your appointment includes a traditional health screening, but our primary goal is to give you the time, tools and support you need to explore your own sense of health and well-being. Your health measures (numbers for cholesterol, blood pressure, etc.) are part of the overall health snapshot you're provided, but we also offer you a supportive environment affirming you as the expert of your own health and well-being. More time and attention is devoted to the conversation-based portion of the screening because you know what matters most to your well-being.

You will receive the single hard copy of your confidential health measures and supporting information at the end of your appointment. Following your appointment, you will be able to access your health measures on your secure UK LiveWell dashboard.

UK HealthCare Employees (hospitals, clinics and clinical areas of the College of Medicine): If you choose to participate in a Check In screening, please attend during part of your lunch period or before or after your regular work shift. Register online now. Or, as an alternative to attending a Check In event, you may submit recent health measures from your physician/provider.

UK Employees (non-HealthCare): We encourage you to attend an upcoming LiveWell Check In before, during or after your regular work shift. If you choose to attend during your work shift, please check with your supervisor in advance to minimize any potential disruption. Register online now. Or, as an alternative to attending a Check In event, you may submit recent health measures from your physician/provider.

Participation eligibility

Generally, eligible participants can participate once every other year. Participants are eligible for one 20-minute appointment during their eligibility window (see question below).

Yes, employees (including STEPS) are eligible to receive the free health screening regardless of UK health plan membership. However, only UK health plan members will earn the rebate.

Yes, during your eligibility window (see above), you may complete an alternate process to earn your rebate by providing documentation from your health care provider in regard to your recent health measures.

In order to receive your rebate, you must complete the appropriate submission process for your UK health plan (either the HMO process or the EPO/PPO/RHP/Indemnity/Medicare process) during the window of time you are eligible for your rebate. Please note the process differs by provider. When choosing your healthcare provider in the LiveWell dashboard, there is the option to select whether your health measures were completed by a UK HealthCare provider or another healthcare provider. This means even if your insurance is PPO, EPO, etc., you may need to select UK HealthCare if your health measures were completed by a UK HealthCare provider.

No. We understand things come up and we're sorry to hear you weren't able to attend your originally scheduled appointment. However, rebates will only be given to participants who attend and complete a Check-In. We encourage you to make another appointment at your convenience or you may also choose to earn your $100 rebate by completing the alternate process which involves submitting documentation from a visit to your provider/physician. 

Employees paid bi-weekly will typically receive the rebate on their UK paychecks within three weeks from the date of completion. 
Employees paid monthly will receive the rebate in the month after completing their Check In (e.g., if the Check In is completed in September, payment will be made in October).
Spouses/adult sponsored dependents of UK employees will receive the rebate on the UK employee's paycheck in accordance with the above timing.
Retirees and retirees' spouses will receive the rebate mailed to their home address in the month after completing their Check In.

UK employees who are on a health plan will receive their rebate on their UK paycheck to streamline the payment process and allow for taxation, which is required.

You must be employed at the University at the time the rebate is paid in order to receive it. Please note the payment timing shown above in "When and how will I receive my UK health plan rebate?".
 

Preparing for your appointment

Please make arrangements to arrive on time or a few minutes before your exact appointment time. Please note that arriving any earlier than 5 minutes before your scheduled appointment is unnecessary. It is unlikely that you will be seen earlier than your scheduled appointment.

Little to no time at all! Please plan to arrive on-time for your scheduled appointment, or 3 to 5 minutes before your exact appointment time.

We understand that things come up and sometimes you cannot avoid running late. We recommend that you still show up and we will do our best to get you in – though if you are more than 2 minutes late you may not be seen close to your originally scheduled time since we must honor the scheduled appointment times for participants arriving on time. If you arrive at any point beyond your scheduled appointment time, please be aware that you may have to wait and/or you may not be able to be seen that day.

Go the UK campus map and click "Find a campus location", "Building Search", and then type in the building name. On the date of your appointment, you will see posted signs near the building. Additionally, signs will be posted inside directing you to the exact location of the Check In event.

Your Check In should last approximately 20 minutes from your scheduled appointment to the time you leave.

Staying hydrated and having warm hands will ensure a quick and easy finger stick (blood test). Drink lots of water on the day of your appointment and consider dressing in layers. We will have a hand warmer on-site to help with this.

No, fasting is not required. It's completely up to you to choose whether fasting (for a more accurate blood test) will work for you on your appointment date. You may choose:

  • Fasting
    For the most accurate glucose and triglycerides readings you will need to fast for 9-12 hours prior to your appointment. If you have diabetes and wish to fast prior to participating, please check with your doctor to see if and how to fast in a way that is best for you.

  • Non-fasting
    We realize fasting isn’t always practical or easy depending on your appointment time, so don’t worry if you choose not to fast. Non-fasting results are accurate within 2% for total cholesterol and HDL. Glucose and triglyceride results will be interpreted based on non-fasting results. Keep in mind that cholesterol, triglycerides and glucose can vary from day-to-day regardless of fasting or not, choosing not to fast does add to this variation. 

If you are concerned about your triglyceride level, talk to your primary healthcare provider who can help interpret your number – he or she knows you best and can tell if this is a pattern and of concern. Note that a non-fasting triglyceride sample may be more representative of the "usual" circulating level of triglyceride since most of the day blood lipid levels reflect post-meal levels rather than fasting levels. However, it is not yet certain how to interpret non-fasting levels for evaluating risk so, at present, there is no change in the current recommendations for fasting prior to tests for lipid levels.

Cancellations

Illness, scheduling conflicts and life sometimes just get in the way! If you are unable to make your appointment, you will be able to cancel your appointment online. Cancellations can be filled on the same day - even during the same hour. Cancel using your LiveWell dashboard.

We are serving a large population with these screenings and we hope to serve you! However, if you miss three appointments without notification, you will no longer be able to make a Check In appointment and will need to choose the alternate process involving completing the health screening portion with your provider/physician to be eligible for your rebate. This policy is only to prevent repeat missed appointments and therefore increase the number of people we reach – your co-workers who are waiting for a spot will thank you!

Privacy

Yes. All information is secured confidentially and your personal health information (PHI) is protected in compliance with the Health Insurance Portability and Accountability Act (HIPAA).

The last step of the online registration process requires a standard “consent to participate” agreement common to all Health and Wellness programs. This UK Health and Wellness staff members supporting you through the Check In (and other programs) to review your confidential results with you for the purposes of follow-up health education, coaching/consultation or disease management. There is a separate “use of health information” question, which allows you to indicate if your health information may be used confidentially for research (your choice does not affect your eligibility to participate or to receive a $100 UK health plan rebate).
