Chronic Disease Self-Management Program

What You Need to Know: 

The Chronic Disease Self Management Program is a free workshop taught once a week for six consecutive weeks. The program encourages a team approach to learning how to deal with symptoms of chronic disease. People with different chronic health conditions - heart problems, arthritis, diabetes, asthma, or depression - attend class together to learn how to deal with symptoms of their chronic disease. If you are living living with a chronic condition, this workshop can help you better manage your symptoms, set goals, and work with your health care providers. It will help you make better choices for better health.

When: Begins August 20 and continues for 6 consecutive weeks
Time: 5:30 p.m. - 7:30 p.m.
Where: UK Polk Dalton Clinic, 217 Elm Tree Lane, Lexington, KY 40507 (free parking available)
