PRIVATE PESTICIDE APPLICATOR FREQUENTLY ASKED QUESTIONS
What do I do with the applicator training forms?
The 5 1/2" X 8 1/2" form has two copies, white & yellow. Give the white copy to the applicator and keep the yellow copy. We no longer send forms to the Division of Pesticides in Frankfort, nor to the area Program Directors. Keep the remaining copy in case there is a need to verify training. The form can be discarded when the certification expires.
What do I do if the applicator does not want to use his social security number?
Encourage the applicator to use the social security number. If not, a unique number with the same number of digits as a social security number must be created, and the number recorded for re-use with the same applicator when re-certifying.

There is no need to send copies to other addresses.
What do I do if someone received training and was not issued a card?
Issue a card based on the training date entered on the applicator training form.
What if someone's card is lost, or destroyed?
Re-issue the card using the original number and expiration date.
Why should I use special procedures when setting up a new computer?
There is a counter used to create the certification number. If the file with the most recent certification number (coini.dbf) is not moved to the new machine, then the counter resets to zero and the certification numbers will be duplicated. The same thing happens if the computer's clock is re-set.
