I am concerned about the health and well-being of my student, what should I do?
If you or someone else are in an emergency situation that requires medical, psychological, or police services, call 911. Once the situation is safe, please fill out the form for one of our staff to follow up with the referred individual.

How do I make a referral to CSI?
Go to any page on CSI's website and click on the "Report a Concern" button to submit an online referral to the CSI staff. If you would like to speak to a CSI staff member about your referral or concern, please call us at 859-257-3755 or email csi@uky.edu.

For off-campus and cell phones, dial 859-257-UKPD (8573). Once the situation is safe, please fill out the form for one of our staff to follow up with the referred individual.
What if I want to make a referral to the Center for Support and Intervention but don't have access to the online referral system?
If you do not have access to the online referral system, please call the Center for Support and Intervention directly at 859-257-3755 or email us at csi@uky.edu.
I have a concern and it is outside normal business hours, what should I do?
If your concern is an emergency, please refer to the first question at the top f the page. If your concern is not an emergency, please submit a referral and the Center for Support and Intervention will respond during regular business hours.
How can I reach the Center for Support and Intervention directly?
Please call 859-257-3755 and ask to speak with a CSI staff member or email us at csi@uky.edu.
What to do if I need to make a referral after normal business hours?
After normal business hours (Monday - Friday 8:00 am - 5:00 pm), please use our online referral system by clicking on the "Report a Concern" button. All referrals will be reviewed by CSI staff the next business day. See the emergency resources page for 24 hour crisis support options.
I don't want the individual that I am referring to know that I submitted the referral about them. Is there an anonymous referring option?
It is possible to submit a referral anonymously however we strongly encourage you to provide your information for follow up on the concerns. If an anonymous referral is submitted, CSI may be limited in the outreach and support that can be provided.
I am submitting a referral for an employee NOT a student. What happens to employee referrals?
Initial responses for employees of concern are addressed by the most appropriate office based on the nature of the referral. Examples include Human Resources, Employee Relations, Office of Institutional Equity and Equal Opportunity, and UK Legal Counsel. If the referral is considered a high level of concern then the referral is discussed at the next CoC Team meeting.
