The Transform Health clinic is a partnership of UK HealthCare providers who have the shared goal of meeting many of the unique medical needs of lesbian, gay, bisexual, and transgender individuals in Kentucky. The clinic is committed to providing the highest quality care in inclusive environments. Clinic services include primary care, preventive care, PrEP, adult hormone therapy, counseling, and tobacco dependence treatment.

To be seen in the Family Medicine Transform Health Clinic, please call 859-323-6371, choose option 1 for appointments, and ask for Transform Health. Initial visits will be with one of the physicians and referrals to the counselor and/or nurse educator will be made at that time as desired or appropriate.

UK COUNSELING CENTER (UK STUDENTS ONLY)

New clients who are seeking individual therapy or to join the Trans* and Gender Non-conforming Support Group at the University of Kentucky Counseling Center (UKCC) must first have an initial assessment. Clients can access same-day walk-in initial appointments in Frazee Hall or can call to schedule an initial appointment at (859) 257-8701 with any clinician.
