STUDENT CONSENT AND OBLIGATION STATEMENT

Please read the following information carefully to understand your rights and responsibilities and to avoid misunderstanding.

 

UNIVERSITY EMAIL

The University of Kentucky’s official method of correspondence with students is email. Official communications from the University shall be sent to the student’s University assigned email address. I understand I am responsible for activating my University assigned email account.

 

CONSENT TO CONDUCT UNIVERSITY BUSINESS ELECTRONICALLY

I understand that by clicking the “accept” button at the bottom of this form, I am agreeing to the terms and conditions provided on this form and entering into agreements and transactions with the University of Kentucky using electronic records and signatures. Agreements and transactions covered by this consent include, but are not limited to, the following:

  • Registration: enrolling, dropping or withdrawing from courses; viewing student schedule, grades and transcripts; ordering transcripts; applying for graduation; requesting and viewing degree audits; FERPA release requests; requesting enrollment verifications; etc.
  • Financial Aid: receiving disclosures and notices; viewing, accepting or declining financial aid; reviewing communications; etc.
  • Student Account: viewing tuition bills; receiving notification of loan disbursements; IRS Form 1098-T Agreement; signing up for Direct Deposit; etc.
  • Miscellaneous: updating contact and emergency contact information; signing statements of financial responsibility; receiving student consumer information; etc.

I understand my consent means that the University is not required to provide me with records in paper format. If I want a paper copy of any record the University has sent to me electronically, I may print a copy of that record or I may contact the University office responsible for the communication, transaction or agreement. However, I understand that even with this consent, the University reserves the right to provide communications and engage in transactions and agreements with me in paper form at any time.

I understand that I may withdraw my consent to do business with the University electronically at any time by clicking this link studentrecords@uky.edu to send an email with my linkblue (LINKBLUE ID) and student ID number (00000000) to the Office of the Registrar. However, I understand that withdrawal of my consent may limit my ability to use myUK, and I am responsible for contacting the appropriate University offices to conduct University business. I understand that declining to provide or withdrawing consent will not impact the University’s official mode of communication with me, which remains through my University assigned email address. University communications will continue to be routed to my University assigned email address, and I will be responsible for all academic and administrative timelines provided to me in this format. I also understand that any withdrawal of my consent to do business with the University electronically does not affect or withdraw my agreement to the other terms, conditions and responsibilities below.

 

TERMS, CONDITIONS AND RESPONSIBILITIES

Student Account Services

  • Billing

I understand the University uses electronic billing (e-bill) as its official billing method, and therefore I am responsible for viewing and paying my student account e-bill by the scheduled due date. I further understand that failure to review my e-bill does not constitute a valid reason for not paying my bill on time.

  • Payment of Fees/Promise to Pay

Request and completion of registration constitutes a contractual financial obligation to pay tuition and fees for which I am liable. I am responsible for reading and understanding the current Drop/Refund policy of the University as it appears in the current Schedule of Classes. Permission to cancel enrollment does not constitute, nor shall it be construed as, a waiver by the University of my financial obligation. I understand that any financial assistance I receive will be applied against my billed charges to reduce my financial obligation.

  • Delinquent Account/Collection

I am responsible for all outstanding debts and contracts with the University. The University reserves the right to assess financial penalties on any indebtedness. Any past-due accounts may be referred to an outside collection agency or the Kentucky Department of Revenue, which could result in collection fees and may be reported to one or more of the national credit bureaus. If my account is referred to an outside collection agency, I understand and agree to reimburse the University for any collection agency fees, which may be based on a percentage at a maximum of 33% of the debt, and all costs and expenses, including reasonable attorney’s fees, the University incurs in such collection efforts. If my account is referred to the Department of Revenue, I understand and agree to pay or reimburse the University for any Department of Revenue fees, which are based on a percentage of the debt, currently 25%, plus 6% interest accrued daily, and all costs and expenses, including reasonable attorney’s fees, the University incurs in such collection efforts.

  • IRS 1098-T Tax Form

I understand that the University is required to report certain qualified tuition and expenses billed to me to the Internal Revenue Service (IRS) on an annual basis on a Form 1098-T. If the University is required to file a Form 1098-T for me, I will be required to provide my social security number (SSN) or other taxpayer identification number (TIN). I understand that failure to provide an SSN and TIN when required may result in a penalty imposed by the IRS and may affect my ability to claim certain federal education tax credits or deductions for qualified tuition and expenses. I understand that the Form 1098-T is provided online through myUK each calendar year.

 

Financial Aid

I understand that I must file the FAFSA annually to be considered for financial aid each academic year. To remain eligible for state and federal financial aid, I understand that I must meet the standards set forth in the Satisfactory Academic Progress Policy.

I understand that my financial aid is estimated until finalized. Financial aid becomes finalized after verification has been completed (when required) and my eligibility is confirmed. Initially, my financial aid eligibility is based on full-time enrollment. Any changes to my FAFSA, enrollment, housing or eligibility status may affect my eligibility and result in adjustments to my financial aid types and amounts.

Based on federal and state regulations and the Financial Aid Overaward Policy, receipt of scholarships, grants, waivers, gift cards, etc., requires adjustments to my financial aid when my total financial aid exceeds certain limits. If I am aware, I will receive additional financial aid funds not reflected in my financial aid package, I must complete and submit the Declaration of Additional Resources Form (DARF).

My annual financial aid offer on myUK outlines each type of financial aid offered with dollar amounts specified by semester/term. I am responsible for clicking on the hyperlink for each financial aid fund offered to me to review its terms and conditions. To receive my aid, I must “accept” each offer fully or partially. If I do not want or need the financial aid fund offered, I must “decline” it. If I wish to reinstate a loan offer I have declined, I must complete the Loan Adjustment Form.

I understand I will be issued a refund by the Student Account Services Office when the financial aid I receive exceeds UK charges.

 

Federal (Title IV) Student Aid Authorizations

Federal (Title IV) Student Aid funds include: Federal Pell Grants, Federal Supplemental Educational Opportunity Grants (FSEOG), Iraq and Afghanistan Service Grants, Federal Work-Study, Federal Direct Subsidized and Unsubsidized Loans, Federal Direct PLUS Loans and TEACH Grants. I understand that any Title IV financial aid disbursed/credited to my account will pay for allowable educationally related charges such as tuition and fees, as well as housing and meals if contracted with the University of Kentucky, for the period for which the funds are provided and up to $200 for previous year charges for tuition, fees, contracted housing and/or meals. I also understand that Title IV financial aid cannot be used to pay for any penalty fees such as late payment fees, late registration fees, return check fees and residence hall/apartment damage fees

I understand that if I answer “Yes” to the authorization indicated below, it will remain in effect until I rescind my authorization or until the end of the academic term. I understand that I may refuse to authorize the item listed below and that I may cancel or modify my authorization at any time by submitting my request in writing to the Student Account Services Office at studentaccounts@uky.edu. Cancellation of this authorization is not retroactive. 

I authorize the University of Kentucky to apply my Federal (Title IV) financial aid for the period for which the funds are provided to pay for Barnes and Noble Bookstore charges I have incurred for books, supplies and personal items such as clothing.

I understand that scholarship/grant assistance in excess of the cost of tuition, fees and books is considered taxable income and must be self-reported for tax purposes according to IRS regulations.

I understand that my financial aid is contingent upon my continued enrollment and attendance in each class upon which my financial aid eligibility was calculated. If I drop any class before completion, I understand that my financial aid eligibility may change, and the financial aid awarded to me may be reduced or canceled. If my financial aid is revoked, I agree to repay those funds. If I decide to completely withdraw from the University, I will follow the instructions provided in the University’s Withdrawal Policy and will familiarize myself with the Financial Aid Withdrawal Policy, which I understand and agree are incorporated herein by reference.

 

Student Records and Registration

  • Privacy Rights and Responsibilities

The Family Educational Rights and Privacy Act (FERPA) of 1974, as amended, is a federal law that protects the privacy and confidentiality of personally identifiable information contained within student education records. The University complies with FERPA confidentiality protections and adheres to procedures dealing with student education records and directory information recommended by the American Association of Collegiate Registrars and Admissions Officers. I am responsible for keeping my University of Kentucky records up to date with my current physical addresses and phone numbers by accessing my student records in myUK.

  • Consumer Information

  • Communication through Email, myUK and Cellular Phones

I agree to access my University assigned email account and my academic, billing and financial aid records that are available in myUK, as well as take care of any actions required of me.

I authorize the University, its vendor partners, collection agencies and its respective agents and contractors to contact me regarding my loan request or my loan(s) and/or other accounts, including repayment of my loan(s) and/or other accounts, at the current or any future number that I provide for my cellular phone or other wireless device using automated telephone equipment or artificial, pre-recorded voice or text messages or personal calls and emails in their efforts to contact me.

Furthermore, I understand that I may withdraw my consent to call my cellular phone using an automatic telephone dialing system by submitting my request in writing to the University or in writing to the applicable contractor or agent contacting me on behalf of the University.

 

ENTIRE AGREEMENT

This agreement supersedes all prior understandings, representations, negotiations and correspondence between the student and the University, constitutes the entire agreement between parties with respect to matters described, and shall not be modified or affected by any course of dealing or course of performance. This agreement does not promise or guarantee that any particular mode of instruction or particular service will be available at all times during the academic term.
