Graduate Program Application Checklist

The application process for the UK Animal Sciences Department requires that several documents be submitted. Some of the documents will be mailed to the UK Graduate School and others will be mailed directly to Dr. Dave Harmon, the Director of Graduate Studies in Animal Sciences. Printable formats of the Animal Science Personal Data Record and Graduate School Application form, reached from the web-site links below, may be filled out and submitted via regular mail. Please see below for mailing addresses and additional info.

	Mail the following materials, denoted by green bullets, directly to the Director of Graduate Studies in the Department of Animal Sciences:
		Completed Animal Science Personal Data Record form. You can either download a printable PDF version of this form or request that a form be mailed to you. Note: You can fill out the form on-line in your PDF viewer and then print the completed form.
		Copies of all undergraduate and graduate transcripts.
		Three letters of recommendation.
		Resume and interests in Animal Science Graduate Program.
		Copy of GRE scores.
		For international applicants: Copy of TOEFL scores.
