Graduate Program Application Checklist

The University of Kentucky Graduate School has recently changed its application process; however, in order for the Animal and Food Sciences Graduate Program to complete our selection process, we also require that the Personal Data Record document be provided to our department.

Below is a list of documents required and where to submit them:

• Completed Animal & Food Sciences Personal Data Record (Submitted to A&FS)
  
  
• Transcripts from all universities attended (Submitted to Graduate School)
  
  
• Three letters of recommendation (Submitted to Graduate School)
  
  
• Resume (Submitted to Graduate School)
  
  
• One page personal statement (Submitted to Graduate School)
  
  
• Copy of GRE scores (Submitted to Graduate School)
  
  
• Copy of TOEFL scores (international applicants) (Submitted to Graduate School)
