County Availability

UK-HMO Lexington Service Area is available in the following
counties: Anderson, Bourbon, Clark, Fayette, Franklin, Jessamine, Madison, Mercer, Scott and Woodford.

UK-HMO Regional Service Area is available in every Kentucky county EXCEPT the following: Allen, Anderson, Bourbon, Boyle, Breckinridge, Clark, Davies, Fayette, Franklin, Grayson, Hancock, Hardin, Henderson, Jessamine, LaRue, Madison, McCreary, McLean, Meade, Mercer, Muhlenberg, Nelson, Ohio, Pulaski, Scott, Simpson, Todd, Union, Wayne, Webster and Woodford.

UK-EPO, UK-PPO, and UK-PPO High
is available in every county within the state of Kentucky.

UK Indemnity is only available to those participants who live
or travel out of state for extended periods of time.
