Graduate School Forms


The Department and programs have adopted a number of forms for administrative purposes. On this page we have collected the several forms and reproduced them in several different styles, whenever possible. The forms are available in Word. Open the form of choice and use the "Save As" command to save the Word file to your desktop. To make sure you are printing the least number of pages, you might want to "view print" and adjust the margins before printing.

Departmental forms

  • Statement of Personal and Professional Responsibility (MSWord) to be completed by September 15.
  • Incomplete ("I") Grade Record (MSWord) to be completed by the instructor and the student, if possible. The document is filed in the Department Office.
  • Quality Assurance Contract (MSWord) to be completed for each paper submitted.
Counseling Psychology Program forms
  • Application for Counseling Practicum (MSWord) to be completed for each practicum.
  • Time Distribution (MSWord) to be completed at the start of each semester.
  • Master's Program of Study (MSWord) to be completed and signed by the Advisor.
  • Educational Specialist Decision Form for Psychological Associate (MSWord) to be completed at the start of the Educational Specialist degree program
  • Educational Specialist Decision Form for Professional Counselor(MSWord) to be completed at the start of the Educational Specialist degree program
  • Independent Study Form (MSWord) to be completed for each Independent Study.
  • Counseling Psychology Program of Study (MSWord) to be submitted at the annual Advisory Committee meeting.
  • Practicum Hours Data Record is to be completed by the last practicum class for each semester. The rationale and an Excel spreadsheet for recording practicum hours is available.
  • Practicum Site Evaluation Form (MSWord) to be completed by the site supervisor and submitted by the last practicum class for each semester.
  • Evaluation of Practicum Site Form (MSWord) to be completed by the practicum student and submitted by the last class for each semester.
  • Educational Specialist Memorandum of Understanding (MSWord) to be submitted to the Major Professor and CPAC Chair by the end of the first month of internship
  • Counseling Psychology Annual Review Form ( MSWord) to be submitted to the Major Professor by the last Friday in March.
  • Doctoral Internship Intent Form (MSWord) to be submitted by second Friday of class.
  • Pre-internship Proposal Verification (MSWord) to be submitted first day of pre-internship class.
  • Memorandum of Understanding (MSWord) to be submitted to the DOT by the end of the first month of internship.
  • Internship Hours Data Record to be submitted with the final internship evaluation. The Excel spreadsheet is available.
  • Internship Evaluation Form (MSWord) to be completed at the end of the internship.
Educational Psychology Program Forms

School Psychology Program Forms

Graduate School forms The forms require Adobe Acrobat Reader. If you have a question about a form contact the Director of Graduate Study (DGS).
