Voluntary Enrollment in Pre-Tax Deductions for Parking Permits

This form may be used if you have already registered for your employee permit but forgot to request a pre-tax deduction. If you would like to have the cost of your parking permit deducted from your paycheck on a pre-tax basis, please enter the information requested. Your UK ID number may be found on your paycheck stub to the right of your name under 'Person ID', or your Social Security Number may be used as an alternative.







"I hereby authorize the University of Kentucky to payroll deduct my parking permit fees on a salary reduction basis (pre-tax). The pre-tax deduction will begin as soon as possible after receipt of my request, and will continue for every month thereafter until my election is revoked or until my permit expires, whichever comes first."
