How do I enroll or make changes?
For open enrollment, there are two options and it's up to you whether you prefer making changes online or submitting paper forms.
Making a change for a qualifying event? This requires submitting paperwork to the Employee Benefits Office within 30 days, though you may be able to upload some documentation online.
1. Online using myUK
You may enroll from your office computer or in the comfort of your own home with your own computer.
It’s easy! Use myUK and go to the employee self-service tab. First, you'll enter your link blue ID and password and click “sign on.” (This is the ID/ password you use to log into your Exchange e-mail account, or in some cases, to log into SAP. For assistance with your ID or password, contact Information Technology at 257-1300.)
Then follow the Quickstart Guide for those benefit plans you need to update.
2. Paper forms
Enroll using paper forms. Pick up forms at 112 Scovell Hall or access a printer-friendly form below:
- Employee Health/Dental/Vision insurance enrollment form
- Employee HSA insurance enrollment form
- Life and AD&D insurance form
- Flexible Spending Account form (health and/or dependent care)
- Retiree Health/Dental/Vision insurance form
Changes you can make any time during the year
There's no need to wait for the next benefits open enrollment to make some changes. You can make the following changes any time throughout the year:
- Decrease optional life insurance coverage
- Adjust voluntary retirement contributions
- Check out UK discounts on auto or home insurance
- Change beneficiaries for your life insurance or retirement plan
When can I enroll in or make changes to my benefits?
According to UK HR Policy #93, "Enrollment may occur within the initial 30 days of employment, during periods of open enrollment or within 30 days of a qualifying family status change.
Qualifying events are listed in the table below, along with the required documentation for each type of event.

A change in enrollment will be effective as of the date of the qualifying event. For example, you may change your benefits five days after the birth of your baby, but your benefits will show the change occurred on the day of birth.
