It's best to discuss your flex work request with your supervisor before submitting this form. Once you submit it, your supervisor will receive an email copy. He or she will reply to the email with "APPROVED" in the subject line and send a copy to you, as well as the Office of Work-Life. If you have not received a response from your supervisor within two days, you may want to ask them about your pending request.

The Office of Work-Life is here to answer any questions you have about flexible work options. We do not approve or deny requests, but we are here to support leaders and employees in implementing and understanding these practices.
