FMMC 10-10 – POWER LIFT SAFETY

PURPOSE

To ensure that all operators of mechanical devices such as fork and scissors lifts are properly training in the safe operation and use of the machines. The training must meet the spirit of OSHA.

POLICY

Only Physical Plant employees trained and certified are authorized to operate forklifts. The training will meet OSHA requirements. Each operator must be licensed/certified to show that they have been trained and tested. The testing may be written and/or verbal. In addition, the operator must demonstrate proficient and safe operation of the equipment, with periodic re-evaluation. This data will become part of the PPDMC database employee training record.

 

Procedure for Fork Lifts:

  1. All operators will receive proper training for safe use and operation to the machines.
  2. A knowledgeable experienced trained trainer will conduct the training.
  3. A copy of the training certificate will be placed in the employees training records.
  4. The person trained will be issued a license showing date of training, number of hours trained, the name of the trainer and the type equipment the license covers.
  5. Performance evaluation will occur every 3 years, adding that date into the employee record.
  6. If an operator is observed operating the equipment in an unsafe or erratic manner, their operator privileges will be suspended until the individual is retrained.
  7. Remedial or refresher classes may be conducted on an as needed basis or if any of the following conditions occur:
    • If a different type of fork lift is introduced to the work place
    • After an accident or near miss
    • After evaluation reveals that the operator is not operating the unit safely
    • If work place conditions change
    •  If operator is observed using the vehicle in an unsafe manner
  8. The forklift must be parked in the designated space when not in use, following our protocol.
  9. Vendors, contractors or unlicensed persons are not permitted to operate or use the equipment.
