Health and Wellness Grant Opportunity

Jul 1, 2016

The Health & Wellness Incentive Program has been designed to provide funds for community organizations to develop or enhance inclusive, accessible health and wellness activities and programs. Approved Projects may receive up to $1,000.00 per project. As a participant in the 2011 Health Matters: Train the Trainer conference, you are being invited to consider this opportunity. Funded sites will provide a final report including the activities undertaken, total number of participants, number of participants with disabilities, impact upon the organization, and future plans to sustain inclusive health and wellness efforts.

In collaboration with consumers and families, the Human Development Institute aims to provide continuing education programs and activities that promote the independence, productivity, and inclusion of people with disabilities and their families throughout life. As a part of this goal, the Institute aims to collaborate with organizations around our state to ensure the active participation of individuals with disabilities in health and wellness programs.

The attached document provides details on grant submission, due no later than January 27, 2012. Please direct any questions to Kathy Sheppard-Jones (email kjone@uky.edu; phone 859-257-8104).
